VIVO works alongside Support Coordinators, Psychosocial Recovery Coaches, Behaviour Support Practitioners and allied health professionals seeking consistent one-to-one disability support across Bundaberg and Wide Bay.
VIVO is currently founder-led, with Cade directly involved in intake and support delivery. As VIVO grows, participant fit and consistency remain central to how additional workers are introduced.
A clear pathway for direct-support referrals and ongoing communication.

Explore ↓Direct support that can work alongside relevant recommendations and strategies.

Referral process ↓When you're sourcing direct support, you need enough information to know whether the provider is genuinely suitable. VIVO aims to make that straightforward: clear scope, honest capacity, direct communication and no pretending a referral is a fit when it isn't.
We understand that your role includes helping participants connect with providers that suit their goals, needs and plan. Our job is to give you a clear picture of what VIVO can actually provide.
Speak with the person responsible for intake and current service delivery rather than being passed through layers of administration.
If the requested hours, needs or location aren't a suitable fit, we'll say so rather than accepting a referral we can't support properly.
Relevant referral information, reports and participant-specific guidance are reviewed to understand the person and the requested support.
With appropriate consent, relevant changes, concerns and useful observations can be communicated to the wider support team.
VIVO's direct-support model is particularly relevant where consistency, relationship-building and understanding the person's wider context matter.
We've borrowed the part of strong referral systems that matters most: professionals should know what is happening with the referral and who to contact.
VIVO reviews the enquiry and makes contact to clarify fit, capacity and the support being requested.
Where supplied and relevant, we review referral information, reports, participant preferences, known risks and agreed support strategies.
VIVO considers the person's needs, requested hours, location, funding arrangement and current capability before accepting support.
Relevant observations or changes can be communicated appropriately with the participant's professional team rather than leaving direct support disconnected from the wider picture.
While VIVO remains founder-led, Cade is directly involved in intake, communication and current service delivery.
Direct support often sits between appointments, therapy sessions and planning conversations. With participant consent and where appropriate, VIVO can help keep everyday support informed by relevant professional recommendations and participant-specific strategies within our role.
Review relevant information provided to VIVO before and during support.
Clarify what relevant recommendations mean within the direct-support role.
Support appropriate participant-specific strategies consistently within capability and agreed scope.
Share relevant observations appropriately with the wider team where consent and circumstances allow.
Where documentation is relevant to the participant's support, VIVO aims for objective, useful records that help show what occurred rather than vague notes that tell the wider team very little.
The exact documentation and information-sharing arrangement depends on the participant, consent, service agreement and what is relevant to the support.
A referral starts a conversation, it isn't an automatic acceptance. We first establish whether VIVO is the right provider for the person and the requested support.
Send the enquiry through VIVO's referral/contact pathway or contact Cade directly.
We review the support requested, location, hours, funding arrangement and relevant context.
We speak with the participant, representative or referrer to clarify needs and what good support should look like.
Capability, capacity, relevant information and service arrangements are considered before commencement.
If the fit is right, introductions and service arrangements are completed and support can commence.
You don't need to have everything perfectly packaged before contacting us. But these details can help VIVO understand the referral faster and more accurately.
Who we should speak with and the preferred contact pathway.
Where support is needed and the approximate schedule or frequency.
What the person wants support with and what they're working toward.
Confirmation that relevant supports are plan-managed or self-managed.
Only information relevant to understanding and safely delivering the requested support.
Anything VIVO needs to understand before deciding whether support can be provided safely and appropriately.
Support Coordinators and other professionals need enough information to help participants compare providers. VIVO won't ask a referrer to present us as the only option or imply that a participant has to use VIVO.
VIVO's role is to explain our services, availability and fit clearly. The participant remains free to choose the provider that best suits them. If VIVO isn't the right fit, we'll say so.
Send through the referral, ask about current capacity, or call first if you want to discuss whether VIVO could be suitable.
